Jersey Kickboxing & MMA — Participant Agreement

1. Assumption of Risk

I understand that participation in martial arts — including but not limited to Boxing, Kickboxing, Muay Thai, Brazilian Jiu-Jitsu, Mixed Martial Arts (MMA), Strength & Conditioning, sparring, grappling, weight training, and related activities (collectively, the "Activities") — involves inherent risks of physical injury, including but not limited to bruises, sprains, strains, fractures, concussions, dislocations, dental injuries, permanent disability, and in rare cases, death. I voluntarily and knowingly assume all such risks, both known and unknown, even those arising from the negligence of the Released Parties (as defined below).

2. Release and Waiver of Claims

In consideration of being permitted to participate in the Activities at Jersey Kickboxing & MMA, located at 600 G Street, Millville, NJ 08332, I, for myself, my heirs, executors, administrators, and assigns, do hereby release, waive, discharge, and covenant not to sue Jersey Kickboxing & MMA, its owners, instructors, employees, agents, independent contractors, affiliates, training partners, landlords, and any other participants (the "Released Parties") from any and all liability, claims, demands, causes of action, and costs whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by me while participating in the Activities or while on the premises.

3. Indemnification

I agree to indemnify and hold harmless the Released Parties from any loss, liability, damage, or costs they may incur arising out of my participation in the Activities, whether caused by the negligence of the Released Parties or otherwise, to the fullest extent permitted by law.

4. Medical Fitness & Authorization

I represent that I am in good physical condition and have no medical conditions that would prevent safe participation. I have disclosed all relevant medical information truthfully above. I authorize Jersey Kickboxing & MMA staff to render or obtain emergency medical treatment on my behalf in the event of injury, and I accept financial responsibility for any such treatment.

5. Rules & Conduct

I agree to abide by all gym rules, instructor directives, hygiene standards, and codes of conduct. I understand that failure to do so may result in immediate termination of my membership or participation, without refund.

6. Photo / Video Release

I grant Jersey Kickboxing & MMA permission to capture and use photographs, video, and audio recordings of me taken during classes, events, and gym activities for marketing, social media, and promotional purposes, without compensation. I may revoke this consent in writing at any time.

7. Membership Terms (Monthly Plans)

Monthly memberships renew automatically on the same date each month via the payment method on file. I may cancel at any time with at least seven (7) days' notice prior to my next billing date. Sign-up fees are non-refundable. Day passes are single-use and non-refundable.

8. Minors

If the participant is under 18 years of age, the parent or legal guardian named above agrees to all terms of this Agreement on behalf of the minor and additionally releases the Released Parties from any claims they may bring individually or on behalf of the minor.

9. Severability & Governing Law

This Agreement is governed by the laws of the State of New Jersey. If any provision is found unenforceable, the remaining provisions shall remain in full force and effect.

10. Acknowledgment

I have read this Agreement in full, understand it completely, and sign it of my own free will. I understand that I am giving up substantial legal rights by signing it.
